In your request please provide the following information:

First and Last Name
User Name that you would like: Minimum 6-10 characters, letters or numbers, no special characters.
Password that you would like: Minimum 6-10 characters, letters or numbers, no special characters.

Please note that at this time only members over the age of 16 will be issued passwords.
